Making Whole Person Care Work: Because Interoperability Shouldn’t Be Aspirational

District leaders aren’t waiting for interoperability to someday materialize—they’re already putting it into practice. In Washington, DC, the Health Information Exchange and hospital community are converting data sharing into meaningful, cross setting care coordination. This panel examines how HIE infrastructure is evolving beyond basic real time alerts to close gaps in transitions of care and connect clinical, behavioral health, and social data in support of whole person care.
