Making Whole Person Care Work: Because Interoperability Shouldn’t Be Aspirational
District leaders aren’t waiting for interoperability to someday materialize—they’re already putting it into practice. In Washington, DC, the Health Information Exchange and hospital community are converting data sharing into meaningful, cross setting care coordination.
