Crystal Eubanks, MS, is Vice President of Care Transformation at the Purchaser Business Group on Health and Executive Director of the California Quality Collaborative (CQC). A leader in strengthening primary care, she leverages expertise in scaling practice transformation and quality improvement, honed through leadership roles across ambulatory care settings. Directing CQC, her statewide efforts have demonstrably improved care—impacting over 20 million Californians in areas like chronic disease and depression management. Crystal previously led nationally recognized transformation efforts at a large community health center and holds an MS in Health Care Quality from George Washington University.
