Diego Emestica currently serves as the Program Manager of Chronic Disease at Northeast Valley Health Corporation (NEVHC), where he oversees patient-centered initiatives focused on diabetes, hypertension, and anticoagulation management. He leads a team of care coordinators and collaborates closely with clinical and operational departments to ensure continuity of care across NEVHC’s primary care centers. His efforts focus on delivering high-quality, integrated services that reduce disease complications and improve health outcomes for high-risk populations.

Before joining NEVHC, Diego worked at Valley Presbyterian Hospital and with nonprofit organizations serving under-resourced communities throughout Los Angeles. His experience in both hospital and community health settings has shaped his deep understanding of chronic disease prevention and management, particularly in underserved populations. He holds a Master’s in Healthcare Informatics from the University of San Diego and is passionate about leveraging data and technology to advance equitable, outcomes-driven care.
